Designed to be to opened, completed, and printed using the Adobe “READER” application, the forms have an additional feature that allows you to save the data you enter. The forms may also be printed blank and completed by hand.
A completed sample submission/test request form must accompany each sample shipment to WuXi AppTec.
If you have any questions about submission of samples or completing these forms, contact the facility where the testing will be performed.
NOTE: Information entered into these forms is not transmitted to WuXi AppTec electronically. The forms serve only to produce the hard copies needed to accompany sample shipments.
